Kim Porter, 47, was found dead at her home in Los Angeles, California, on Thursday.
A rep for the starlet told The Hollywood Reporter: “Sadly, I can confirm the passing of Kim Porter. I ask that you give the families privacy at this time.”
The model and actress famously dated Sean ‘Diddy’ Combs, 49, for thirteen years, and the couple share three children together — Christian, 20, and twins Jessie and D’Lila, 11.
Kim also has a son named Quincy Brown, 27, — an actor — born from a previous relationship.
According to multiple US reports, police were called to Kim’s house at around 11.30am, where officers found her unresponsive.
While the actress’ cause of death is not yet known, TMZ reports she has been suffering from pneumonia or the flu for many weeks.
A rep for Diddy confirmed the sad news to the publication, asking that the family be allowed privacy.
Kim and Diddy started an on/off relationship in 1994, before parting ways for good in 2007.

Following news of Kim’s passing, fans have taken to social media to pay tribute to the starlet.
One sympathiser tweeted: “All I can think about are her babies. Kim Porter seemed like such a genuine, loving, involved, and encouraging mother.”
They added: “Sending prayers to all who knew and loved her. This is so sad.”
Another wrote: “Four children lost their mother. My heart breaks for the Combs children. Keep them and those close to their family in your prayers as they mourn the loss of their matriarch, Kim Porter.”
Meanwhile, rapper and actor 50 Cent, 43 — real name Curtis Jackson — tweeted: “RIP to Kim Porter, I send my condolences to her friends and family.
“She was loved and will be missed dearly. I know Puff [Diddy] is hit right now, he loved her for real, soul mate type s***. Chin up, Puff. Positive vibes only.”
US talk show host Wendy Williams, 54, paid her respects, writing: “I just heard the sad news about Kim Porter. My condolences to Diddy and Kim’s families. Rest In Peace.”
Rapper Missy Elliot, 47, posted: “This is so heartbreaking. Lord, please give her kids and her whole family strength,” alongside a praying hands emoji.
